Platelets are key factors in hard and soft tissue repair mechanisms. The stimulation of fibroblasts and endothelial cells induces new extracellular matrix deposition and neo-vascularization. Plasma is essential for cell survival as it contains nutrients, vitamins, hormones, electrolytes and proteins. Proteins are key molecules for the coagulation process and the formation of the fibrin polymer that will serve as a scaffold for cell migration, differentiation and proliferation.
Although the system is technically capable of producing significantly higher platelet concentrations, it is not what research shows to be beneficial for clinical use. More and more studies demonstrate that concentrations of platelets 1 to 3 times over the baseline show more robust healing rates than those with concentrations of 3 to 8 times the baseline 1. Some studies even showed that too high platelet concentrations may actually have negative effects.
In a in vivo study 2 pedicon 2021 was shown that highly concentrated platelet preparations had an inhibitory influence on osteoblast activity, probably due to unwanted inhibitory and cytotoxic effects of growth factors at such high concentrations. These substantial differences are rooted in divergent approaches to centrifugation. Regenkit relies on the simplicity of single spin preparation while EmCyte PRP further concentrates platelets with a dual spin approach. Both kits rely on the principles of differential centrifugation to isolate platelets.
While under the radial force of centrifugation, the various components of whole blood are separated by weight. The heaviest elements, red blood cells, travel at a faster rate and settle at the bottom of the kit followed by white blood cells, platelets followed by plasma at the top.
Emcyte Pure PRP kit protocols call for the initial supernatant to undergo a secondary centrifugation. During this subsequent centrifugation blood elements become more clearly stratified and additional platelets fall out of the plasma solution.
The secondary centrifugation yields PRP serums with higher platelet recovery rates. Single spin PRP kits rely on centrifugation as well as a gel polymer to stratify blood elements. Our scientific advisor suspects the polymer to have a specific gravity just above white blood cells. This is reasonable to deduce because of the extremely low granulocyte and hematocrit levels found in Regenkit PRP serums.
The polymer settles above red and white blood cells and the lighter blood elements, which excludes nearly all of the red and white blood cells from the injectate.
Low hematocrit levels can benefit patients by decreasing pain at the injection site.
In-House Regen Med Labs: Great Idea or Dangerous?
Outside of vascular pathways, red blood cells cause oxidative stress and patients report a stinging sensation. Though Rengenkit eliminates potential pain caused by red blood cells at the injection site, the final serum may not contain ideal concentrations of platelets.
In fact, third party research found that Regenkit PRP serums contained fewer platelets than whole blood, technically platelet poor plasma. Platelets are non-nucleated fragments of megakaryocytes. Megakaryocytes are large progenitor cells located in the bone marrow that extend into sinusoidal blood vessels to release platelets into the bloodstream.
Platelets are not uniform in size. Unfortunately, it seems that a majority of platelets have a higher specific gravity than the gel polymer and are likely to be trapped within or settled below the polymer. Platelets have growth factors that act like espresso shots for cells that can repair tissue. A key thing that we have always seen is that the number of platelets in the mix matters. Hence, we use high-dose PRP that generally has much higher platelet levels than those produced by a simple bedside centrifuge. At Regenexx, we never want to be backed into a corner by one technology. If you read this blog you know that I often write about what I experience. The vast majority of doctors use bedside kits to process platelet-rich plasma, bone marrow, or fat.
The latter is much more complex and also much more flexible. Meaning with an in-house regen med lab you can customize the sample more and create higher concentrations of cells than bedside machines. However, for the vast majority of doctors who have these labs in their practices, none of those controls exist.
In fact, for them, what they know about processing came from a video or a weekend class. The message that kicked this blog loose from my brain came from a medical provider who witnessed the following at a clinic with an in-house processing lab:. First, this is obviously a fly by night operation. They had sterility issues as PRP should only be processed in a Biologic Safety Cabinet and not at the bedside in an open fashion using the wrong equipment. They took out all of the platelets in the test tube and injected platelet-poor plasma while telling the patient they were injecting platelet-rich plasma.
The doctor had no idea of the difference, even though he or she is the last link in that Quality Assurance chain. Meaning, they took a weekend course or watched a video on how to process and there is no on-going development or testing even when the variables in the processing change. Compare that to Regenexx, where we have a lab and clinical research team where we spend almost a million USD a year.
On what? In part on continuously learning how to improve cell processing protocols. We also continuously test our assumptions about what we produce in our flexible lab platform and create new minimally manipulated orthobiologics to help our patients. Finally, we perform clinical research to ensure that what we produce works in real patients. Processing blood or bone marrow takes specialized equipment like a Biologic Safety Cabinet BSC that you usepipettes, sterile tubes, special centrifuges, cell counters, etc… However, not only does it take special equipment, but you have to make sure the equipment works as advertised.
It has HEPA filters that clean the air that need to be periodically changed. The hood itself must be validated annually through special tests. Using the hood and cleaning it takes a specialized process. At Regenexx, all of our labs are required to have their equipment validated by outside 3rd parties. Does the staff know how to use it? Is it working properly? Have the counts been validated against manual counts? At Regenexx we do all of this, at a clinic with an in-house lab, who knows? This is basically a highly detailed manual for everything done in the lab, from processing patient samples to cleaning the hood, to emptying the trash.
Almost none of these labs I see out there have SOPs that would survive even the most basic lab audit. The pic shown here is a tiny fraction of these that I was updating with our lab manager at Centeno-Schultz. However, having the documents is one thing, but you also need to test the processors on these documents to make sure they understand how to use them.
